Web Publisher Authorization Form
Filling out the Web Publisher Authorization form in any format, indicates that you agree and will adhere to the agreements. Please read the following statement prior to submitting a request.
The School District of Philadelphia maintains a World Wide Web service as an expression of its mission to educate children. It is expected that the resources available through this service will enhance the academic and social development of students and contribute to the professional development of teachers. In addition the Web Service will be a source of information about the school district.
Access to the District’s Web Service is considered a privilege that is based upon fulfilling the agreements stated below.
Director’s Agreement
- I will monitor the content of my department’s web pages in accordance with applicable laws, the Pennsylvania Education Code and School District policies.
- I will supervise the person I name below as my department’s website editor/web publisher.
- I will contact the Webmaster of the Information Technology Department regarding any access privilege or technical matter.
Web Publisher’s Agreement
- I will publish the content of my department’s web pages in accordance with applicable laws, the Pennsylvania Education Code and School District policies.
- I will inform the principal/director named above of the content of my department’s web pages.
- I will contact the Webmaster of the Information Technology Department regarding any access privilege or technical matter.
- I will not give the password that gives me access to my department’s space on the District’s Web Service to anyone.
