Transferring a Child from a Philadelphia
Public School
You will need to bring the following:
An EH25-Notification of Transfer slip from the previous school
2 proof of address--see below for acceptable proof
A copy of the immunization record
Registering a Child for the
First Time
The following items must be brought with you at time of registration:
1. Verification of Date of Birth
Birth Certificate (preferred document)
or Hospital Record or Baptismal Certificate
2. Record of Immunization
Pennsylvania State Law requires children to have the following
vaccines:
- DTP - 4 or more doses. DT 4th dosage must be after 4 years
of age
- Polio Vaccine - 3 or more
- Measles, Mumps, Rubella, (MMR) - 2 doses, 1st dose after the 1st
Birthday-
- 2nd dose no sooner than 1 month after 1st
dose.
- Hepatitis B-3 doses, however your child may be admitted with provisional
status.
- Your child must have the 1st dose to register for school and must
complete the within 8 months of school entry to remain in school.
- Varicella Shot- or the date your child had the chicken pox.
Please note: ALL IMMUNIZATION DATES MUST INCLUDE month,
date and year.
3. Proof of Residence
Utility or cable bill, Department
of Public Assistance card, paycheck with printed
address, driver license, car insurance,
voter’s registration, deed or mortgage
payment book. TWO PROOFS
OF ADDRESS IS REQUIRED. ONE MUST BE A UTILITY BILL. LEASES AND
PHOTOCOPIES OF BILL ARE NOT ACCEPTABLE
4. Child’s Social Security Number
5. Emergency Information
The name and phone number of someone we can
contact when you are available and work phone number of parents.
6. A Doctor completed physical and dental exam forms is required for
Admission to Kindergarten, (which you will receive at time of your
appointment)
YOU MUST HAVE THE FIVE ITEMS ABOVE WITH YOU AT TIME OF REGISTRATION,
OR WE CANNOT REGISTER YOUR CHILD.
