Greetings from the school nurses at Central High School!   The Nursing Department is committed to enhancing the health and well being of all students.  We look forward to working wh you and your child to promote their optimum level of health so that they may succeed here at Central.  We encourage you to take the time to read our entire page so that you are well informed about the services and care we provide.

Early Dismissals:

Only listed emergency contacts or parent/guardians can pick up a child for early dismissals due to medical reasons. Make sure that your emergency contact form is on file.  Contact your child’s Advisor or Main Office for emergency contact forms or download from the school district’s website and return to the main office.  As per School District policy all early dismissals must be done in the Main Office with proper identification.

Health Requirements and Compliance for Students:

All ninth graders (9th) are required to bring in a completed physical exam form including immunizations by their doctor to the nurse before being admitted to school. Exams may be completed any time after September 1st of the prior calendar year.  Click on Physical Examination Form (MEH-1) below to print this form to take to their doctor.  Click on Required State Law Immunizations below to update student’s record accordingly. Medical exemptions for immunizations must be in writing from student’s doctor and Religious exemptions must be in writing from the parent.  Students with chronic health problems such as asthma, diabetes, allergies, etc. must provide an update on their illness, using the Student Health Status Form (S865).  Click on the form below to be completed by the parent/guardian. All students requesting medication(s) prescribed or over-the-counter to be administered during school hours are required to have a Request for the Administration of Medication in School form (MED-1) completed by his/her doctor and signed by the parent for each medicine to be dispensed.  Click on the Medication Authorization Form below to print this form.  Students without a dental examination on file will be asked to provide one.  Click on Report of Private Dental Examination (MEH 155) below to be completed by their dentist.

RESPONSIBILITIES & HOURS

As most of you are aware the School Nurses have multiple responsibilities.  These include performing state mandated health screenings (vision, hearing, height and weight), maintaining student health and immunization records, administering and/or supervising the administration of doctor prescribed medications, performing nursing procedures, responding to school illnesses and accidents, assessing and interviewing students with serious health problems. We process and monitor medical Homebound and medical Transportation request. We also process and monitor medical 504 plans (service agreements).

Due to our aforementioned responsibilities if your child has a non-emergent problem they will be expected to wait until our open hours.  Please be aware that acetaminophen or ibuprofen will not be dispensed before 11:00 AM or after 2:00 PM.

All students must have a note to come to the Nurses Office except for emergencies.

11:00 am – 12:00pm Open for routine health problems

12:00 pm – 1:00 pm Closed for lunch and case management

1:00 pm – 2:00 pm Open for routine health problems

2:00 pm – 3:00 pm Closed for documentation and follow up
