You will need to provide documents verifying your home address as well as student’s age. Please collect all materials before moving to the next step.

 

Proof of your student’s age
Examples of this include birth or baptismal certificate and medical records with student’s date of birth

Two (2) documents showing your address
Examples of this could be a lease, deed, or utility bills with your name and the address in which you reside

Shot (Immunization) Records
1. Obtain this from your child’s physician if you don’t have a copy already
2. Use the above link to read The School District of Philadelphia’s policy on vaccination records

Parent or Guardian photo identification (requested but not required for registration)
Examples of this could be drivers license, passport, or state photo identification card
