Pre-Employment

EHS approves and files all School Personnel Health Record forms (also referred to as a physical form).  An approved physical form is required to complete the pre-employment process. All prospective School District of Philadelphia employees are required to submit a current physical form to meet Pennsylvania regulations.

The physical form includes a tuberculosis/PPD test and a physical examination. If you don’t have a personal physician, click the quick link for a list of Philadelphia Health Centers.

Please note: Pennsylvania law requires that your tuberculosis/PPD results be read within 48 – 72 hours of the test application date. Your tuberculosis/PPD results must appear on your School Personnel Health Record. Make sure your doctor or health care provider fills in this section completely and accurately and signs the box next to the results.

 

Bus Chauffeur Physical

All School District of Philadelphia Bus Chauffeurs and other employees who possess a commercial drivers license (CDL) must have a complete physical examination before hiring and then annually after hiring.
