Oral Health Integration Program

Cigna Dental Oral Health Integration Program® offers enhanced dental services for individuals with qualifying conditions and for pregnant women. It’s easy to enroll. If you have heart disease, history of a stroke, diabetes, chronic kidney disease, have had an organ transplant or had head and neck cancer radiation, lupus or rheumatoid arthritis, complete the Cigna Dental Oral Health Integration Program® Registration Form and fax it to 859-550-2662 or mail to Cigna Dental P.O. Box 188037 Chattanooga, TN 37422.

NOTE: the SDP plan also includes Periodontal Treatment & Maintenance for Members with LUPUS and Rheumatoid Arthritis.

How do I know if my dentist is participating?

You can ask your dentist if they participate with the Cigna DPPO network, or you can contact Cigna 24/7 at 800.564.7642.  They can answer questions about your benefits and help you find participating dentists.

It’s easy to search for a dentist on Cigna.com.  Visit the site and click “Find a Doctor, Dentist or Facility.”  Follow the prompts on screen and when asked to choose your plan, select “DPPO/EPO > Total Cigna DPPO.”
