Voluntary Benefits

Product offerings include Critical Events, Accident, Whole Life, Identity Theft, Legal Assistance and Pet Insurance.

PFT represented employees are ineligible to participate in these benefits at this time.

These benefits are optional but you may find that they are appropriate to address your needs. There are no changes to your core benefits, and electing these benefits has no impact on those enrollments.  Active employees can enroll during Open Enrollment May 1 through May 31 at http://mysdpbenefits.com/.

Cancellation

Accident, Critical Events Insurance, Whole Life, ID Theft and Pet Insurance can be cancelled at anytime. The Legal Assistance product can only be cancelled during the Open Enrollment period May 1 through May 31.

Cancellation requests for Accident, Critical Events Insurance, Whole Life, ID Theft  should be done by contacting Winston Benefits and completing cancellation paperwork.  It may take 2 pay cycles for the cancellation to be processed and the payroll deduction to be stopped in your paycheck. To cancel Pet insurance, contact Nationwide directly at 1-877-738-7874.

CRITICAL EVENTS INSURANCE:

Serious illness can be costly. Transamerica’s Critical Events Insurance keeps your finances healthy while you recover. You’ll receive a lump sum payment if you are diagnosed with a covered illness (including cancer), which you can use however you see fit.

Policy Features:

  • Obtain coverage for yourself, your spouse, or your children
  • Employees choose $10,000 or $20,000 Guaranteed Issue Coverage
  • Spouse/Domestic Partner will be offered 50% and child(ren) will be offered 50% of employee benefit amount
  • No health questions or physical exams required
  • Coverage is portable: you may take your policy with you if you leave the District

Additional Benefit Riders:

  • Cancer Benefit Rider
  • Recurrent Benefit Rider (50%)
  • Wellness Benefit Rider: $50 per covered person (employee & spouse only) per year

No pre-existing condition limitations but benefits are not payable for losses incurred prior to the effective date.

ACCIDENT INSURANCE:

Accident Insurance provides benefits for covered injuries and specific accident-related expenses for an individual or family. Since your health insurance plan only covers certain expenses with applicable plan limits, this Transamerica policy is designed to cover out-of-pocket expenses that result from a sudden accident, such as deductibles, co-payments, and non-covered medical services. You’ll receive a lump sum payment that you can spend as you choose. Employees must be scheduled to work a minimum of 20 hours a week in order to be eligible for coverage.

Policy Features:

  • Obtain coverage for yourself, your spouse, and your children
  • 24-hour coverage – paid benefits for accidents that occur on and off the job
  • No health questions or physical exams required
  • Coverage is portable – you may take your policy with you if you leave the District

  • Expanded Benefits Rider

WHOLE LIFE INSURANCE:

Whole Life Insurance can help you protect your family against unexpected death by giving them financial protection with cash value that grows over time. You can purchase Whole Life Insurance for yourself, your spouse and/or your children. Coverage amounts greater than Guaranteed Issue requires an Evidence of Insurability approval from Transamerica. Coverage Amounts:

Employee:

  • Guaranteed Issue: Lesser of $150,000 or 5x Annual Salary
  • Maximum Amount of Coverage: Lesser of $500,000 or 5x Annual Salary
  • For the first 10 years of coverage, employees that have enrolled for less than the Guaranteed Issue amount will be allowed to increase coverage by $1 per week, up to a maximum of $5 per week, not to exceed the full Guaranteed Issue limit.

Spouse:

  • Guaranteed Issue: $50,000 not to exceed 50% of employee’s amount.  If Employee does not enroll or enrolls for less than $30,000, Spouse Guaranteed Issue will be $15,000.
  • Maximum Amount of Coverage:$100,000

Child(ren):

  • Child(ren)/Grandchild(ren): Guaranteed Issue- Whole Life: $25,000
  • Optional Child(ren)Term Life Rider: Child(ren) only: $10,000

Policy Features:

  • Guaranteed Issue – no medical exam required
  • Level premiums never change
  • Guaranteed growth of your money every year (minimum interest rate of 4%)
  • It’s portable – you can keep the coverage if you leave the District

ID Theft happens every two seconds. To help you and your family protect your identities, we offer you Identity Guard Value Plan with Watson.

PET INSURANCE:

Seventy percent of households in the United States own pets – and if you’re one of them, you know how costly health care for your pets can be. Nationwide Pet Insurance provides coverage for accidents, illnesses, and diseases, and includes a wellness care option. You have the freedom to use any licensed veterinarian, including specialists and emergency providers. What’s more, it’s portable – you can take it with you if you leave the District.

Plans offer a choice of reimbursement options so you can find coverage that fits your budget. All plans have a $250 annual  deductible and $7,500 maximum annual benefit. Coverage includes*:

VOLUNTARY TERM LIFE INSURANCE:
In addition to the benefits we currently offer, you have the option to purchase additional term life insurance through convenient payroll deductions. You can also enroll your spouse and children in policies of their own if you participate in the plan. The Hartford Voluntary Term Life Insurance plan is offered on a guaranteed issue basis up to $150,000 without proof of good health within 30 days of hire. A request for Life Insurance after the initial enrollment period (31 days after hire) requires an Evidence of Insurability approval from The Hartford.

Coverage Amounts:

Employee:

  • Guaranteed Issue: $150,000
  • Minimum Amount of Coverage: $10,000
  • Maximum Amount of Coverage: $1,000,000

Spouse:

  • Guaranteed Issue- $30,000
  • Minimum Amount of Coverage: $5,000
  • Maximum Amount of Coverage: $250,000(Cannot exceed 100% of employee amount)

Child:

  • Guaranteed Issue: $10,000
  • Minimum Amount of Coverage: $2,000
